Safety culture and safety leadership matter, but they are the foundation, not the destination. Too many organizations are treating them as the finish line.
Here's what I've observed. Organizations with award-winning cultures and celebrated leaders are still experiencing serious injuries and fatalities. That should tell us something. Culture doesn't stop a critical control from failing. Leadership visibility doesn't compensate for a system that can't detect deviation before it becomes a tragedy.
The next leap won't come from doing more of the same, better executed. It will come from organizations that get precise about what culture and leadership are actually supposed to produce: System capacity, Critical control integrity, Recovery strength.
Culture is the soil. System capacity is the crop. Most organizations are obsessing over the soil while wondering why they keep losing the harvest.
